In this position, the Care Coordination Program Manager assumes responsibility for coordinating all clinic, wellness, and laboratory operations onsite, aligning them with regulatory and compliance standards as well as with our company mission and values. Additionally, they actively collaborate with services to ensure our members have access to the best resources available. Beyond these responsibilities, the Care Coordination Program Manager serves as the manager of our internal Care Coordination department. They provide guidance and support, going above and beyond to surpass the continuum of care needs of our members. By excelling in this multifaceted role, our Care Coordination Program Manager ensures that New Freedom's commitment to delivering exceptional member care and experiences remains unwavering. Your Impact Will Include: Serve as primary liaison with thirdâ??party medical, MAT, lab, and wellness providers to support member outcomes. Coordinate care with external partners and internal departments to ensure seamless service integration. Provide direct oversight to Care Coordination Case Management staff and promote health and wellness practices. Manage resources and workflows across care coordination services within New Freedom facilities. Develop, track, and update reports, metrics, and systems that support care coordination operations. Support staff with problemâ??solving, conflict resolution, coaching, and performance development in partnership with leadership and HR. Ensure accuracy and compliance in EMR documentation, regulatory requirements, policies, and procedures. Contribute to quarterly goalâ??setting, maintain accountability toward KPIs, and participate in required supervision and continuing education. Performing additional duties as assigned to support our mission. Where You’ll Work Primarily onsite at our offices. Requires occasional travel to program sites/facilities Requires flexibility to respond to emergent situations, after-hour calls, or urgent department needs that arise Using standard office equipment, plus plenty of face-to-face interaction.
